Affordable Care and Insurance

We accept a growing list of insurance plans. We also offer affordable cash-pay appointments to those with a need.

Insurance Status*​

Note: Some insurance companies are starting to no longer accept telehealth services. We don't always know this until after we submit your bill (since they have the right to change it any time they want). It's up to you to check and understand your coverage options for telehealth services.

* Please note that insurance companies have many sub-plans and networks. If you want to be certain that your plan is covered, we can check for you, please contact us with your name, address, plan ID, and group number.

​Our Fees:

We offer longer and more affordable appointments than many of our peers in the area. Contact us for self-pay rates if you don't have accepted insurance.

  • ​90-minute new patient intake – Based on your insurance. If you'll be paying out of pocket, contact us for slide scale rates or payment plans.

  • 30-minute follow-up – Based on your insurance. If you'll be paying out of pocket, contact us for slide scale rates or payment plans.

  • Gender-affirming surgery and medical letters are FREE to current recurring patients. We also offer a limited number of monthly free appointments for non-patients seeking an external assessment and documentation for gender-affirming medical care. Contact us for details and support.

  • No Show fee (see practice policy for definition, cancelations are required 24 hours in advance) – The full price of the scheduled appointment type will be charged to your credit card on file.

  • Paperwork completion outside of an appointment (e.g., FEMLA forms, school forms, etc.) Charged in 15-minute increments at $50 per 15 minutes.

Payment:

Copays are due at the time of the appointment. If you have a deductible, you may be asked for a deposit at the start of the appointment.  Self-pay appointment payment is due at the start of all appointments. (For Self-Pay - we provide you with a “superbill”). We do not allow patients to carry a balance, all fees must be paid prior to the next appointment.

We accept payment by all forms of digital payment including FSA/HSA cards (with a Visa or Mastercard logo).
